<p>Estrogen and testosterone are very similar in composition and can  both effect target tissue similarly.  In fact, estrogen is produced from  testosterone. Within the ovaries a reaction occurs in which the  testosterone (with 19 carbon atoms) interacts with an enzyme that acts  to remove a single carbon atom from the compound, producing estrogen  (with 18 carbon atoms).</p>
<p>We have also described the absolute importance of estrogen on genital  tissues to prevent genital atrophy. Both systemic estrogen (oral pills,  transdermal patches, transdermal creams) and topical vaginal creams can  supply adequate replacement estrogen to the genital tissues if needed.  However, for these you need the assistance of your gynecologist.</p>
<p>Estrogen is also necessary for the adequate clitoral blood flow  needed to establish sexual arousal and a clitoral erection. Since an  orgasm can only be achieved from the continued stimulation of a  completely erect clitoris, estrogen is necessary for optimal sexual  arousal of the clitoris. This is done so that they can study the effects of  the testosterone medication with the assurance that every participant  had similar estrogen levels. <p>Viagra® (sildenafil) was originally developed as a treatment for high   blood pressure.  This  drug aimed to reduce blood pressure by causing  blood vessels to dilate  and loosen up. However, during the clinical  trials many men refused  to return the left-over medicine due to its  potent ability to cause a  penile erection.</p>
<p>L-arginine comes primarily from dietary intake, and is the only  starting  point for the Nitric Oxide Synthase pathway. When the  electrical signal  of sexual arousal activates the NOS pathway,  L-arginine becomes  converted into nitric oxide. Nitric oxide acts as a  vasodilator and  promotes yet another more powerful element,   cyclic-GMP, a very potent  vasodilator.</p>
<p>What results is a build-up of  this potent vasodilator thus creating a  penile erection. Though the  producer of this drug (Pfizer  Pharmaceuticals) has tried to prove this  drug effective for women,  there have been little positive results. The reason that sildenafil  cannot be proven to be beneficial  for women is probably due to the fact  that the NOS pathway is very  sensitive to, and in need of adequate  testosterone and estrogen.  Remember, women are complex!</p>
<p>While the Nobel Prize in Medicine was not awarded for the knowledge  that  to have an orgasm it is essential to become maximally sexually  aroused  with a clitoral erection, the discovery of the science  underlying  genital erection did. The physiology that enables a woman to  establish a  clitoral erection (and a man a penile erection) is the  Nitric Oxide  Synthase Pathway.  The discovery of this pathway was the  basis of the  1998 Nobel Prize in Medicine.   Now we know that the NOS  pathway  explains how a signal originating in the brain and traveling  via the  nerves (esp. the clitoral nerve) causes the release of nitric  oxide, the  key event that results in clitoral engorgement and clitoral  erection.</p>
<p>This NOS pathway relies on the presence of L-arginine, a  semi-essential amino acid that is usually contained in most cells of the  body (esp. blood vessels). It is this amino acid that causes the  dilation of blood vessels, an important step resulting in  vaso-congestion and thus an erection.  While it is not L-arginine itself  that acts directly on the tissue, L-arginine and only L-arginine makes  this possible by its conversion into other compounds.</p>
<p>This induced production of two important vasodilators is like  throwing gasoline onto a smoldering fire, causing a large and rapid  reaction. This NOS  mechanism creates maximum sexual arousal and  clitoral erection by inducing the activity of nitric oxide with  L-arginine.</p>

<p>All of sexual arousal initiates in the brain.  Dr. Arcos states:    “In women, the cycle of sexual response begins in the brain, where a  memory, an image, a scent, music or fantasy acts as a trigger to prompt  sexual arousal.”</p>
<p>The fact is our brain is the main sexual organ and its where all the  magic of sex and intimacy begins.  The nerves in the brain and spinal  cord causes a chain reaction, leading to direct stimulation of the  pelvic nerves that lead to the vagina and clitoris.</p>
<p>The initial stimulation of the pelvic nerves causes an enlargement of  the blood vessels that supply the vagina and clitoris. Genital  stimulation creates the sensation of pleasure in the brain, which then  acts to increase electrical action of the pelvic nerves to increase  vaginal and clitoral blood vessel changes.</p>
<p>A woman’s sexual arousal is not like the ON/OFF switch of a light,  but rather like a car rolling down a steep hill. Just like how a car  rolling down a hill builds up speed with time, a woman’s sexual arousal  builds in intensity over a period of time.</p>
<p>The progressive and increasing stimulation  of the pelvic nerves causes these very specialized nerves to release  small pockets of vaso-active intestinal polypeptides (VIP – first  described from the intestines). These cause the blood vessels that  surround the vagina to dilate (causing more blood flow).</p>
<p>Both the small arteries (arterioles), that supply fresh blood to the  tissue as well as the small veins (venules), that drain blood from the  tissue become dilated. What results is an increased amount of blood  arriving at the tissue, while at the same time even less blood being  allowed to drain away. The net effect that occurs is a tissue engorged  or saturated with blood. This is called vaso-congestion, and it is the  basis of all sexual arousal both in the vagina and in the clitoris.</p>
<p>With vaso-congestion the vessels that surround the vagina become  permeable (porous) and allow the fluid component of blood to saturate  the vaginal mucosa in-between the cells.  This fluid component of blood  accumulates in the lumen of the vagina and provides the vaginal  lubrication.  Vaginal lubrication is the initial vaso-congestive  response of a woman’s sexual arousal.</p>

<p>Last time we talked about the brain and the 2nd Sexual Revolution…</p>
<p>The term “clitoral erection” cannot be found in the medical  literature before the 1990’s.  The knowledge and understanding that a  woman’s clitoris must become fully erect so we can have orgasm is the  basis of the Second Sexual Revolution for Women.</p>
<p>As with men, they cannot achieve orgasm without a penile erection;    This knowledge is becoming well known to women; but few men understand  how a woman achieves orgasm, and the importance of an orgasm to women.   Once again, sexual satisfaction is defined and achieved only by an  orgasm.</p>
<p>So what it Key to the Kingdom -Vaso-congestion is the key!Vaso-congestion  is the medical term that describes the action where more blood is going  in to an organ than is released. Just as in the male penis, the female  clitoris has an internal structure termed the ‘corpus cavernosa.’ (In  Latin: Corpus=Body, Cavernosa=Caves.)</p>
<p>Think of it as a sponge.  In a non aroused state, the sponge is dry;  in the sexually aroused state the sponge fills with blood and grows to  several times its original size.  This vaso-congestion causes the  clitoris to increase in length and diameter and protrudes erectly  outside of the clitoral hood.</p>
<p>This is a cold “hard”  fact(no pun intended) as it  allows for a  higher degree of physical stimulation during sexual activity.  The most  important result of an erect clitoris is that the sexual sensory nerves  become hypersensitive to touch -  ladies you know what I am refering  to!   This hypersensitivity of the clitoral nerves allows a women to  achieve orgasm.</p>

<p>When doctors do not completely understand something, they use this  word called  ‘dysfunction’ (such as in Brain Dysfunction, and Female  Sexual Dysfunction).  Within the diagnosis of Female Sexual  ‘Dysfunction’, there are 3 specific categories:</p>
<ul>
<li> Hypoactive Sexual Desire Disorder</li>
<li>Hypoactive Sexual Arousal Disorder</li>
<li>Orgasmic Disorder or Anorgasmia  –  “An” or “A” before a word means without.   Anorgasmia is defined as the lack of an orgasm.</li>
</ul>
<p>For those of us who have to work at it, sexual satisfaction is  actually having orgasm. The only way to have one is to have enough  interest, sexual desire, and sexual arousal.  These are the trifecta for  toe-curling, mind-melding orgasms.  When one of these are missing, we  begin to think something is wrong.</p>
<p>To diagnose sexual dysfunction, the FDA requires that the reported  lack of desire, lack of sexual arousal, or lack of an orgasm causes  distress for an individual woman.  The FDA needs a medical diagnosis to  be able to evaluate the effectiveness of a specific drug (when compared  to placebo) in treating the symptoms which cause the dysfunction or  disorder.  Speaking from experience, “distress” – reads as frustration  &amp;  despair.</p>
