Estrogen

Estrogen is important for:
1) Youthful Genital Tissues
2) Optimal Sexual Arousal

Estrogen and testosterone are very similar in composition and can both effect target tissue similarly.  In fact, estrogen is produced from testosterone. Within the ovaries a reaction occurs in which the testosterone (with 19 carbon atoms) interacts with an enzyme that acts to remove a single carbon atom from the compound, producing estrogen (with 18 carbon atoms).

We have also described the absolute importance of estrogen on genital tissues to prevent genital atrophy. Both systemic estrogen (oral pills, transdermal patches, transdermal creams) and topical vaginal creams can supply adequate replacement estrogen to the genital tissues if needed. However, for these you need the assistance of your gynecologist.

Estrogen is also necessary for the adequate clitoral blood flow needed to establish sexual arousal and a clitoral erection. Since an orgasm can only be achieved from the continued stimulation of a completely erect clitoris, estrogen is necessary for optimal sexual arousal of the clitoris. Further, estrogen is also necessary for the lubrication of the vagina, which is a significant component of a woman’s sexual arousal.

Estrogen is important to establish a maximum clitoral erection not only by improving blood flow, but also by optimizing the nitric oxide synthase (NOS) pathway to dilate the corpus cavernosa . The all important NOS pathway that establishes a clitoral erection is dependant upon both estrogen and testosterone. Testosterone is probably more important than the estrogen, but remember they are both very similar and related to one another. In fact, in every clinical study to test the ability of testosterone to increase a woman’s sexual desire and sexual arousal, researchers first make sure the participants are on estrogen replacement therapy. This is done so that they can study the effects of the testosterone medication with the assurance that every participant had similar estrogen levels. Remember, every woman is unique and complex!

The “G” spot explained…

A woman’s anatomy also affects how she experiences intravaginal sexual intercourse.  The inside of the vagina, the vaginal mucosa, has no sensory nerves. This is why the vagina can withstand the vigorous activity of intercourse.  The ‘G-spot’  is the only area where there are sensory nerves in the vagina and is only sensitive to stimulation when the clitoris is fully sexually aroused.  The ‘G-spot’ is located one to one and a half inches inside of the vaginal introtis, beneath the anterior vaginal wall.

Special Sauce in Sensua! – L-Arginine

Viagra® (sildenafil) was originally developed as a treatment for high blood pressure.  This drug aimed to reduce blood pressure by causing blood vessels to dilate and loosen up. However, during the clinical trials many men refused to return the left-over medicine due to its potent ability to cause a penile erection.

L-arginine comes primarily from dietary intake, and is the only starting point for the Nitric Oxide Synthase pathway. When the electrical signal of sexual arousal activates the NOS pathway, L-arginine becomes converted into nitric oxide. Nitric oxide acts as a vasodilator and promotes yet another more powerful element,  cyclic-GMP, a very potent vasodilator.

What results is a build-up of this potent vasodilator thus creating a penile erection. Though the producer of this drug (Pfizer Pharmaceuticals) has tried to prove this drug effective for women, there have been little positive results. The reason that sildenafil cannot be proven to be beneficial for women is probably due to the fact that the NOS pathway is very sensitive to, and in need of adequate testosterone and estrogen. Remember, women are complex!

It’s All In Our Heads…

All of sexual arousal initiates in the brain.  Dr. Arcos states:   “In women, the cycle of sexual response begins in the brain, where a memory, an image, a scent, music or fantasy acts as a trigger to prompt sexual arousal.”

The fact is our brain is the main sexual organ and its where all the magic of sex and intimacy begins.  The nerves in the brain and spinal cord causes a chain reaction, leading to direct stimulation of the pelvic nerves that lead to the vagina and clitoris.

The initial stimulation of the pelvic nerves causes an enlargement of the blood vessels that supply the vagina and clitoris. Genital stimulation creates the sensation of pleasure in the brain, which then acts to increase electrical action of the pelvic nerves to increase vaginal and clitoral blood vessel changes.

A woman’s sexual arousal is not like the ON/OFF switch of a light, but rather like a car rolling down a steep hill. Just like how a car rolling down a hill builds up speed with time, a woman’s sexual arousal builds in intensity over a period of time.

Where is the Clitoris???

Last time we talked about the brain and the 2nd Sexual Revolution…

The term “clitoral erection” cannot be found in the medical literature before the 1990’s.  The knowledge and understanding that a woman’s clitoris must become fully erect so we can have orgasm is the basis of the Second Sexual Revolution for Women.

Sex Dysfunction Explained, Maybe…

When doctors do not completely understand something, they use this word called  ‘dysfunction’ (such as in Brain Dysfunction, and Female Sexual Dysfunction).  Within the diagnosis of Female Sexual ‘Dysfunction’, there are 3 specific categories:

  • Hypoactive Sexual Desire Disorder
  • Hypoactive Sexual Arousal Disorder
  • Orgasmic Disorder or Anorgasmia  –  “An” or “A” before a word means without.   Anorgasmia is defined as the lack of an orgasm.

For those of us who have to work at it, sexual satisfaction is actually having orgasm. The only way to have one is to have enough interest, sexual desire, and sexual arousal.  These are the trifecta for toe-curling, mind-melding orgasms.  When one of these are missing, we begin to think something is wrong.

Why No Viagra for Women

Men are simple!  If a man can achieve a penile erection and has sex, he has an orgasm about 95% of the time and is quite  satisfied.  This is why the yearly sales of Viagra (sildenafil), Levitra, and Cialis combine to almost three billion dollars.  Men are simple, and it is easy for men to achieve sexual satisfaction as long as they have a penile erection.

Women are complex! Several inter-connected factors impact our ability to achieve sexual satisfaction, including estrogen, testosterone, and progesterone levels.  These hormone levels are affected by age, oral contraceptives, and some antidepressant medications, and can heavily influence a woman’s normal sexual response. (Sexual Desire, Sexual Arousal including vaginal lubrication and clitoral erection, Orgasm!)

To be able to market a medication like Viagra¨ to women, clinical studies must be done that prove, to the FDA (Food and Drug Administration) and the medical community, that the medication is safe and effective.  Effectiveness is always measured against a placebo (sugar pill).   For women, Viagra could not prove any effectiveness over placebo in several large clinical studies, which were commissioned and paid for by Pfizer, the pharmaceutical company that produces Viagra.  One study did however show that Viagra¨ can improve a woman’s sexual responses; but only if her estrogen, testosterone, and progesterone levels were optimized using hormone replacement therapy along with the medication.  Further, it was only effective when hormone levels were equivalent to that of a 20 year old woman in the prime of her Sexual Health – not much help for us older women.

The 3 Sexual Revolutions

The First Sexual Revolution
